Step 1

Open your photo into photoshop programs

correct color and contras level with

Images > adjustments >Hue > Saturation

and 

Images > adjustments > Levels

and

Filter > Blur > Lens Blur

Step 2

Create duplicate layer 

add noise for new layer

Filter > Noise >Add Noise

Step 3

From Tools Panel, select eraser tools

Step 4

From Tools Panel, select rectangular  Marquee Tools 

Create square shape up - down part of picture

File with black color ( #000000 )
