STEP 1

Create a new document 300 px x 300 px

STEP 2

Create a new layer and go to Rounded Rectangle tool, create shape with Radius 40 px

STEP 3

Create Text with Horisontal Type Tool

STEP 4

Create a new layer and repeat step in STEP 2

STEP 5

Add layer mask and fill with Gradient Tools

color : Black to Transparant

STEP 6

Repeat STEP 4 and STEP 5  with Ellipse Tool

STEP 7

Add shadow shape

Layer > Layer Style > Drop Shadow

STEP 8

Add Text with Horisontal Type Tool
